US-based patient engagement platform Force Therapeutics has partnered with integrated digital health platform Xealth to simplify remote care monitoring for clinicians while using their existing workflows.
The US-based patient engagement platform leverages AI algorithms, provider-driven protocols, and cumulative intelligence to create personalised care plans for patients.
Under the partnership, the Force platform will integrate with providers’ electronic health record (EHR) system to optimise physicians’ workflow.
The integration would enable care teams to monitor patients’ postoperative data and remote behaviour directly from the EHR and make more informed decisions.
Force Therapeutics founder and CEO Bronwyn Spira said: “This partnership gives clinicians the immediate insights they need into their patients’ daily behaviour.
“Providers can monitor patients’ usage of the Force platform within the EHR, which will help them track care plan compliance and identify at-risk patients who need additional education or follow-up care, without having to log into multiple digital tools.”
The integrated platform leverages event-based triggers to automatically prescribe the Force platform, eliminating the need for physician intervention.
It enables care teams to track patient data, platform engagement, patient-reported outcomes, and physical therapy records, using the patient’s chart.
In addition, the integrated platform would also enable clinicians to monitor patients’ progress towards their recovery, in the future.
The partnership, which brings digital health into provider workflows, will help clinicians practice effective care management at scale, said Force Therapeutics.
Spun out of Providence St. Joseph Health (PSJH) in 2017, Xealth enables clinicians to integrate, prescribe and monitor digital health tools for patients.
