Canada-based Bayshore HealthCare has introduced a remote patient monitoring (RPM) service called Vantage aimed at transforming the healthcare delivery system.

Vantage is a cloud-based solution created in partnership with Microsoft. It is said to leverage the capabilities of Microsoft Cloud for Healthcare.

According to Bayshore HealthCare, the remote patient monitoring programme is intended to enhance patient outcomes, improve healthcare delivery, and streamline communication between healthcare providers.

Vantage is expected to enable healthcare professionals to remotely monitor health of patients, deliver prompt interventions, and bring down avoidable hospital readmissions and visits to emergency departments.

Built on the HL7 FHIR data model, the Vantage platform also ensures easy interoperability with the current healthcare infrastructure, said Bayshore HealthCare.

The Canadian healthcare technology firm stated that the platform supports patients, caregivers, hospitals, and home and community care providers by streamlining remote and in-person patient onboarding through its easy-to-use mobile app and medical devices.

It enables caregivers to monitor the patient’s progress via a single point of contact for clinical and technical support.

Additionally, Vantage offers electronic referral and patient progress monitoring along with customisable care plans and goals.

According to the healthcare service provider, the service is more than just an RPM software programme and is backed by the clinical staff at the company.

Bayshore HealthCare’s nurses are said to closely monitor each patient’s development, interact with their family members and other carers, and quickly react to any changes in a patient’s condition.

Bayshore HealthCare business transformation director Dheeraj Paul said: “Vantage is a game-changer for the healthcare industry.

“By empowering patients, caregivers, and healthcare providers with real-time health data and tools, we’re making it easier for them to collaborate, make informed decisions, and create better patient outcomes.”

Vantage uses adaptable technology from the Bayshore Digital Experience Platform, a new launching pad for enabling the development of a collection of digital home healthcare programmes and services to assist those with chronic illnesses.

This platform also enables collaboration on digital projects with different healthcare organisations across Canada, Bayshore HealthCare said.
