New Jersey-based Kennedy Health System, an acute-care hospital, has selected the ExpressMD Solutions Electronic House Call (EHC) remote vital signs monitoring solution as part of a program to reduce hospital readmissions for Congestive Heart Failure (CHF) patients.

The ExpressMD EHC remote patient monitoring solution improves patient care and reduces costs by delivering results to healthcare providers securely via the Web without requiring an in-person physician or hospital visit.

The easy-to-use in-home EHC touch screen device delivers better continuity of care to specific patient segments that require regular monitoring of medical conditions.

Automated reminders are designed to increase patients’ compliance with care plans, diets, medication schedules and follow-up physician visits.

The EHC can also be used to deliver disease-specific patient education as needed.

Healthcare providers can view each patient’s vital statistics and make adjustments to care plans online, which are then communicated to the patient via the EHC device.
